If your child or teen is an “active” patient in our practice, this page is to help you manage routine communications with your child’s clinician(s) and our office staff. If your child is experiencing a medical emergency, you need to go directly to the nearest emergency room. Our physicians can be reached through the answering service at the number listed in the practice policies and procedures you signed at your child’s initial visit.
Contact our office staff: If you need to cancel or reschedule an appointment, have a question about your account, or have another request for our support staff, please click here. Support staff are working remotely during COVID. Phone messages are typically checked once daily.
Prescription requests: Unless your physician recently started your child on a new medication or adjusted the dose of a medication since your last appointment, you should have received prescriptions for enough medication to last to your next appointment. Click here to request a prescription. If you need a follow-up appointment , our office staff will contact you to schedule one.

Release of information: Click here to download a release of information if you’d like records to be shared with other professionals involved in your child’s care. Please initial (do not check) all lines on the release that pertain to your child and all lines on the bottom of the first page as “YES” since this information is contained in your progress notes. On the second page, please indicate “YES” to all categories that pertain to your child, sign and date.
